Why piggyback progress notes?

Some offices like to separate treatment notes from fee information. These forms provide that style of documentation.

This form functions in similar fashion to the C-101R Dental Treatment Record - Progress Notes, except that the fee column has been eliminated. Document your patient's treatment in detail, but exclude financial particulars. Use alone or with C-401R Confidential Notes Form. Order separately from Starter Kit
