This quick and easy-to-use Emergency Record #CF-ER
provides for adequate documentation for "one time only" patients. It
contains a patient information, medical history, general consent,
diagnosis and initial findings, and progress notes section. It also
contains a pocket for radiographs or "loose" items. It can be used with
the First Encounter chart system
(simply leave in the "open" format, use the pre-perforated holes at the
top, and file on the bottom of the regular forms in the system). Or it may be
used on it's own. Folded-it measures 8 1/2 by 5 1/2 inches and can be
filed in a drawer or on a shelf. Opened-it measures 8 1/2 by 11 inches.
High quality and inexpensive. Available shrink wrapped in packages of 25.
