Even though patients don't like filling out forms, it is the dentist's responsibility to routinely update a detailed health history. This form makes updating health histories quick and efficient, and eliminates patient complaints about excessive paperwork. It provides space for documenting ten visits including blood pressure and patient and dentist signature.

Use the Dental and Medical Histories Updates C-113L form for the patient's initial examination and up to six recall appointments. Use this Medical Updates C-213L form for up to ten subsequent appointments. Order separately from STARTER KIT.
